Your doctor checks your cholesterol, says the LDL looks fine, and you both move on. There is a better number sitting on the same blood draw. It predicts heart attacks more accurately, and almost nobody orders it.
It is called ApoB. The short version: standard cholesterol tells you how much cholesterol you are carrying, ApoB tells you how many particles are carrying it, and it is the particles, not the cholesterol, that wedge into your artery walls.

Weight versus headcount
Your standard LDL-C is a measure of weight. It tells you how much cholesterol is riding around in your LDL particles. ApoB is a headcount. Every atherogenic particle that can lodge in an artery wall carries exactly one ApoB tag, so measuring ApoB counts the particles themselves.
That distinction is the whole issue, because atherosclerosis is a numbers game. It is driven by how many particles are burrowing into your artery walls, not by how much cholesterol each one happens to be hauling. Counting the particles beats weighing the cargo.
When “normal” cholesterol lies
Here is the trap. You can have a perfectly normal, even low, LDL-C and still have a high ApoB. It happens when you carry a lot of small, cholesterol-poor particles: same total cholesterol weight, many more particles, much higher risk. Doctors call it discordance, and it is common in exactly the men this newsletter is for, the ones with insulin resistance, high triglycerides, or a growing waist.
So the guy with “great cholesterol” can be walking around with an artery-particle count that says otherwise. When LDL-C and ApoB disagree, the risk follows ApoB.
The evidence is not close
This is not a fringe position. In a recent systematic review, ApoB was the more accurate predictor of cardiovascular disease than LDL-C in nine of nine studies, and beat non-HDL-C in most of them (strong evidence). It is backed by long-term population studies, by genetic analyses, and by the statin trials. In 2024 the National Lipid Association put out an expert consensus moving ApoB to the center of risk assessment. The science has been ahead of the standard checkup for years.
The numbers to know
Ask for ApoB by name, because it is cheap and rarely offered. Then know roughly where you want it. The 2024 targets get stricter as your risk rises: under about 90 mg/dL if your risk is intermediate, under 70 if it is high, under 60 if it is very high. For a man playing the long game on his arteries, lower is better, full stop.
The levers to move it are the ones you already know: cut the refined carbs and drop visceral fat (the same moves that fix insulin also lower particle counts), keep training, and if your number is high enough, the medication conversation with a real doctor is a legitimate tool, not a failure.

My Experience
For years my cholesterol panel was the one part of my bloodwork I felt smug about. LDL in the good column, doctor nods, I strut out feeling immortal. It was the number I used to justify all the meat.
Then I started asking for ApoB, mostly because I kept reading that the number I was proud of was the wrong number. And ApoB was less flattering than my tidy LDL had led me to believe. Not a disaster, but a clear “you are not as bulletproof as your cholesterol told you” correction. Turns out I had the exact setup this issue warns about, where the weight looks fine and the headcount does not, and I walked into it while feeling superior about my steak intake.
I am on TRT, which can nudge lipids around, so I have a second reason to watch the number that actually maps to my arteries instead of the one that just flatters me. Now ApoB rides on every panel I run, right next to the fasting insulin, because both of them keep telling me things my “normal” results were politely hiding.
The lesson is the whole brand in one embarrassing lived example: normal is not optimal, and the number that makes you feel good is not always the number keeping you alive. I spent years admiring the wrong stat. Do not be me.
